If you’re facing health issues, Food problems, or want Medicare assistance due to a low-income stream. SCMT can help you in this regard!
We provide Health, Food, and Medicare Assistance services to the needy individuals of our community who live in Middle Tennessee or nearby areas. In the respective service, we cover all the expenses utilized by a needy individual for the above-mentioned necessities.

Health Assistance:

The Health Assistance service is here to help eligible low- and middle-income Somali families get prescription medications at no cost, even if you have insurance.

  • Access to medical equipment.
  • Assistance with enrollment in state and federal programs, including obtaining insurance through the Vermont Health Connect exchange.
  • Advocacy and support with obtaining other healthcare-related resources.

Food Assistance:

Food insecurity – living in a household that does not have consistent access to enough food for a healthy, active life – and obesity are increasingly seen as related problems. SCMT Food Assistance program helps Somalis resolve their food-related problems due to their low-income stream and heavy household expenses. We have also provided assistance to the Somali families to enroll in the state/federal level food programs to get them access to their basic necessities of food. We prevent them to steal and misguide by the wrong individuals and create a tough time for them and their families. We provide the facility to think out of the box without worrying about their day-to-day increasing food expenses.

Medicare Assistance:

Our Medicare Assistance Program is part of a statewide team. Our highly trained volunteer counselors are here to help beneficiaries find answers, troubleshoot problems, sort through their options, and understand the Medicare and Medicaid programs. We can help you in:

  • Identify resources for prescription drug assistance
  • Explain Medicare health plan options
  • Understand doctor bills, hospital bills, and Medicare Summary Notices
  • Understand Medicare eligibility, enrollment, coverage, claims, and appeals
  • Enroll in Medicare savings programs
  • Review your Medicare supplemental (“Medigap”) insurance needs, compare policies, and pursue claims and refunds
